Supporting Poplar’s Mission: Enabling Providers to Spend More Time with Patients while Growing Revenue
Poplar is a public benefit corporation with a network of value-based primary care practices across the United States. Connective Health is a digital health company that enables providers to improve outcomes through insights and interoperability. Poplar and Connective Health recently partnered to support new patient onboarding and more accurate risk adjustment across the healthcare system.
Challenge: Progressive Health, a primary care organization within the Poplar network, was grappling with time-consuming chart reviews and incomplete medical records. They were also looking for risk adjustment solutions.
Solution: Connective improves the chart review process and provides insights to providers in their workflows. The solution is based on real-time access to external provider records nationwide, summarized with risk adjustment and care gap algorithms.
Impact: In just one day, Connective Health’s solution streamlined chart review for 39 patients, uncovering 10 new diagnoses and HCCs from external providers. The new diagnosis/HCCs were validated with supporting lab, imaging, and medication information from Connective Health:

- Four others, including breast cancer, cognitive disease, blood disease and musculoskeletal disease
Result: Extrapolated across the year, the time saved and medical accuracy is meaningful, helping to achieve Poplar’s mission of “Enabling Providers to Spend More Time with Patients while Growing Revenue”
